The most common violation is one that you might not witness! On deliveries, accreditors are writing up drivers who mishandle cylinders. 

Drivers should be trained to handle cylinders properly in order to prevent injury and cylinder damage. Cylinders should never be juggled on the way into a patient's home. Instead, drivers should use a delivery bag or cart to carry mulitple cylinders into the home. This not only saves drivers from making multiple trips, but also makes it much less likely that a cylinder will drop out of the driver's arms and injure the driver, patient or damage the cylinder.
